Three things follow.
Use a practitioner profile correctly. Google allows a public-facing doctor to have a profile. A solo doctor at a branded practice should have one profile, named in the format "Practice name: Doctor name". Duplicate or abandoned doctor profiles split reviews.
Pick the specific category. Both doctors in the pack used "Family practice physician", not a general label. A dermatology or cardiology practice should do the same with its own specialty.
Choose the neighborhood before the keyword. "Dermatologist philadelphia" gets about 1,500 searches a month in the United States with a difficulty score of 62. "Urgent care philadelphia" gets about 1,300. "Primary care doctor philadelphia" gets about 60. Most patients search without a city name at all, from home, and see what is near them.
