Local SEO for physical therapy clinics is the work of ranking a clinic in the Google Maps top 3 for patients who are choosing a therapist themselves, with or without a referral in hand. It is for independent, therapist-owned clinics with a verified Google Business Profile at a real location that want more direct bookings and less dependence on a few referring offices.

"Physical therapy near me" gets about seven times the searches of "physical therapist near me".
In every state, patients have some form of direct access to a physical therapist without a physician referral. The American Physical Therapy Association reports that all 50 states reached some form of direct access by 2015, in two levels today, access with provisions and unrestricted access (APTA).
Many patients do not know this. They assume they need to see a doctor first, and some clinic websites never tell them otherwise.
Say it on the home page and in the profile description, in the terms your state allows. For example: "You can book an evaluation without a referral. Some insurance plans still require one for coverage, and we check that for you before your first visit." Two things in that sentence need checking before you publish: your state's direct access level, and how the plans you accept handle it.
A patient with a referral still chooses. A referral for physical therapy is usually not tied to one clinic, so a referred patient often searches "physical therapy near me" to pick the closest or best-reviewed one.
Category names come from a published list of current Business Profile categories (Dalton Luka). Google's guidelines say to use as few categories as possible and the most specific that fit (Google Business Profile guidelines).
Techniques such as dry needling have no category of their own. They belong in the profile's services list and on a page of their own.
Categories are one field. A clinic profile has six more that change how often it is chosen, and the GBP management bundle sets them in the first month and keeps them current.
Google sometimes creates a profile for an individual therapist. Claim it, point it at the clinic's phone and site, and do not ask patients to review it.

Patients fit therapy around work and school, so early morning and evening slots are when they search and when they attend. In late 2023 Joy Hawkins of Sterling Sky showed open businesses outranking closed ones, and Google's search liaison confirmed that openness had recently become a stronger local ranking signal (BrightLocal).
If the clinic treats from 6:30 a.m. to 7 p.m., the profile should say so. Many clinic profiles list front-desk hours of 8 to 5 and give that edge away.
Physical therapy has an advantage most health practices lack: a patient is in the clinic two or three times a week for a month or more, and knows the therapist by first name.
Ask at discharge, or at the visit where the patient hits a goal they set at the start. Ask every patient the same way and offer nothing in return. The FTC's review rule bans incentives tied to a positive review.
Replies must not confirm the reviewer was a patient or mention an injury, even when the review does. The federal settlement over review replies that most health practices cite involved a New Jersey practice that paid $30,000 in June 2023 (HHS Office for Civil Rights). A therapist who has seen someone twelve times can ask in person: "If the care here helped, would you say so on Google?" Follow with a text carrying the link that same day. Under the program, 1- and 2-star reviews are flagged for removal when they violate Google's policies. A fair complaint about scheduling or a copay stays, and the reply invites a call to the clinic without confirming a visit. HIPAA applies to a clinic's public reply even when the patient has posted the details.
Physical therapist is one of the health categories Google lists for Local Services Ads in the United States, with state licenses, malpractice insurance and a National Provider Identifier among the requirements (Google Local Services Help).
Apple Maps, Bing Places and Yelp need the same name, address, phone and hours as the Google profile. Clinics that changed names after a buyout, or that once shared a suite with a gym or a physician office, often have an old listing still showing the former name.
The program tracks 10 keywords on a 5-mile community grid or 20 on a 10-mile city grid. Each point on the grid records the clinic's rank for one search from that spot.
For most clinics the 5-mile grid is the honest one. A patient who comes two or three times a week picks a clinic close to home or work, so rank three miles out matters more than rank ten miles out. The exception is a specialty such as pelvic floor or vestibular therapy, where patients will drive, and those terms deserve a wider look.
Read the grid for "physical therapy near me" first. Gaps usually sit around a competitor's front door, and a gap near a large employer or a school is the one worth an area page. The monthly report gives average rank, coverage, top 3 share, calls, website clicks and direction requests. The program also includes local engagement signals on priority keywords.
